Innovative Model Carefirst One-Stop Multi-Services Centre is a one-of-a-kind facility in Ontario, having adopted an innovative service model, namely Program of All-inclusive Care for the Elderly (PACE), where seniors can access to a basket of care services under one roof, ranging from community and personal support to medical and health care services.
By advocating this PACE model, Carefirst has committed to provide a continuum of efficient, adaptive and thorough services, so as to help seniors live quality and enriched lives.
I am excited that this new Centre with 52,000 square feet service area will commence operation in mid-2015. Yet, we still need your generous support to realize our dream of building such integrated care system for our seniors in the community.

High Functionality Inside Carefirst One-Stop Multi-Services Centre, there will be a Social Services Wing offering a great variety of wellness activities to keep seniors joyful and healthy.
Besides, Carefirst Family Health Team will set up a 18,000 square feet Medical Wing to deliver topnotch medical services. Our multi-disciplinary team of family physicians, specialists, nurses, dietitians, social workers and others will also be the backbone of a Chronic Disease Management Centre to provide patients with health care education and counselling services.
As well, Carefirst will provide patients discharged from hospitals with short-stay accommodation and care services in our Transitional Care Wing before their recovery at homes.
